Unicare Human Services, Inc. provides non-medical home care.  A Unicare representative will meet with the client and address their care needs.  Following our initial assessment, we will work with the client and family members to structure a service plan that best suits their needs.  On a quarterly basis, a case manager visits each client for quality assurance and care plan maintenance.  Care needs are re-evaluated at that time.  Each care plan is updated with the necessary modifications, following approval of the client and their representative or family.

• PM care and repositioning
